Questionnaires

Please see questionnaire procedures below.
 
1.Mail or submit personally at our office (12964 SW 133rd Court - Miami FL 33186) the necessary forms and payments payable to: FPMS (Company Check, Cashier Check, or Money Order ONLY)
In your selected amount depending on the desired turn around time:
$250.00 = $250.00  - 10 Business Days
$350.00 + $100.00 = $350.00 - 3 Business Days

Disclaimer - The questionnaire form MUST be provided with the association name and property address along ATTACHED WITH THE PAYMENT. Payment must specify the property address on the memo.

2. Make Payment Online
Click on link https://payorportal.revopay.com/index.php/enb/properties/fpms/qpay
For address type in the association name with the account number -- "ABC 123"
Click continue to type in your information.
Next you're going to click Questionnaire and one of the two payments options:
               - $250.00 = $250.00 for a 10- business day turnaround
               - $250.00 + $100 (rush fee)  = $350.00 RUSH 3- business day turnaround  
Type in the payment method.
Please be sure to attach the condo questionnaire form with the receipt to this email, estoppel@myfpms.com. 

Please note that if there are two (2) different questionnaires, two (2) processing payments will be required for processing.
